Speech Pathology

At Lift Health Group, our speech pathologists support individuals of all ages to communicate effectively and manage challenges with eating and drinking. Functional Capacity Assessments

NDIS Functional Capacity Assessments (what is)

A Functional Capacity Assessment (FCA) is a comprehensive evaluation of a person’s current functioning to determine that they have the right level of support and funding to maximise their independence and achieve their goals.
 
Lift Health Group provide comprehensive Functional Capacity Assessments to support NDIS funding. We pride ourselves on detailed, timely reports that consider holistically the participant’s needs, taking into account their background history, informal supports, current services, living arrangements, level of engagement within the community, and their goals and aspirations. If applicable, we also complete home and living assessments, to support housing goals and transitions into Supported Independent Living (SIL), Individualised Living Options (ILO), and Specialist Disability Accommodation (SDA).
 
Our Functional Capacity Assessments include recommendations for Core supports, Capacity Building therapies and Support Coordination. We also determine if there is specific equipment, technology, or home modifications required to improve safety and independence. After the Functional Capacity Assessment, further assessments and follow up may be required with our specialised team depending on the needs of the participant, such as;

What is assessed?
Our expect disability assessors will complete a comprehensive assessment, including evaluating a participant’s medical history, disability information, supports, and daily living needs. We review functional domains such as mobility, self-care, cognition, communication, sensory processing, and community participation, using standardised tools like WHODAS 2.0, PEDI-CAT, and Vineland-3. Challenges, risks, and behaviours of concern are documented alongside NDIS goals. Recommendations include core supports, capacity-building therapies, assistive technology, and tailored interventions to improve independence and quality of life.
 
Who can do a Functional Capacity Assessment?
Our team of Disability Assessors include Occupational Therapists and Physiotherapists, as per the NDIS guidelines for suitable allied health professionals. Our team are extensively trained, and we take a collaborative approach to our reports. All of our reports are reviewed within our team for quality assurance, and reports are co-signed to ensure recommendations are optimal for the participant to achieve their goals and maximise their independence.
 
How long should a Functional Capacity Assessment take?
Initial assessment times vary depending on the complexity of the case. Appointment times vary between 1.5 – 3 hours. Report writing time then varies between 10-20 hours.

Continence Assessments

Continence Assessments provide a tailored approach to supporting people through the life stages with bladder and bowel control issues. Continence assessments are conducted by pelvic health physiotherapists and continence nurses to identify specific needs, create personalised management plans and improve quality of life.

Assistive Technology Assessments


At Lift Health Group we are passionate about empowering our clients to be as independent as possible. Through assistive technology assessments clients are supported to access equipment and/or devices that facilitate engagement in meaningful activities of daily living.

Following the receipt a referral, clients will be contacted to arrange a specialised assessment by one of our therapists. This will be tailored to best suit the client’s needs and could be provided by an Occupational Therapist, Physiotherapist or Speech Pathologist. These assessments can take anywhere from 1-4 hours depending on several factors including (not not limited to); complexity of requested items, number of items required and level of risk involved.

To ensure a comprehensive service is provided our therapists will support you through every step of the process, continuing to provide support until our clients and their families/carers feel confident to safely operate the prescribed equipment/device.

The team at Lift have built relationships with local equipment providers in order to ensure that living in a rural area does not mean our clients receive lower quality care. We understand that every individual is different and therefore strive to provide a holistic and comprehensive service to ensure each client’s needs and preferences are considered and they have the opportunity to trial various devices to ensure they are receiving something that meets their needs.

At Lift, we understand that sourcing equipment in rural and remote areas can be challenging, our goal is to ensure that everyone has access to these services, regardless of their location. In the past we have travelled to remote areas outside of our usual locations to deliver and fit assistive technology.

Types of AT we commonly prescribe include (but are not limited to) the following:

  • Mobility devices (e.g. wheelchairs, powered mobility devices, walkers)
  • Falls prevention devices (e.g. pendant alarms, sensor mats)
  • Communication aids (e.g. AAC devices)
  • Daily living aids (e.g. aids to supporting feeding, dressing, sleeping, showering, seating/transfers)

Home Modifications Assessments

At Lift Health Group, we understand the importance of a safe and accessible home environment. Our goal is to support clients in maintaining their independence through tailored home modifications that enhance safety, mobility, and daily living.

Our team specialises in assessing and facilitating a wide range of modifications, working closely with contractors to ensure solutions are practical and aligned with individual needs and preferences.

A home modification assessment typically takes 1–3 hours, after which your Occupational Therapist will:

  • Provide necessary diagrams and reports,
  • Liaise with builders,
  • Prepare NDIS documentation, and
  • Oversee implementation, ensuring you feel informed and supported throughout the process.
